Medical Records for SSA DBL

I have appt with SSA in early August to file for SS disability. A book I got from library and a SSA site suggest contacting my doctors and requesting my records so I have them to take with me to appt.
Letter from SSA confirming appt has form to file out giving them permission to get these records.
I asked a doctor yesterday for these records and they said they wait for the official SSA inquiry and not give them directly to me.
I just thought it may speed the process up if I had records with me opposed to waiting for SSA to contact doctor's, have doctors copy and send them, etc.
Any thoughts on this?

    Hi Kelli,

    Part of the process of filing for Social Security Disability is to complete an Adult Disability Report along with the application. In that Adult Disability Report are sections for contact information for your doctors as well as tests performed. I completed the ADR online and completed the application (which is separate from the disability report)in person with the SS rep.

    Social Security will use your doctor contact info to get the information they need. Your time may be well spent on just concentrating on the accuracy of the contact information for your doctors and which doctor scheduled what tests.

    You will also want to print, sign and bring the Medical Release form with you to the appointment. They can access the Adult Disability Report you complete online when you are there with them. That ADR will speed up the process during your appointment.

    Once your application is filed, then A Social Security Disability analyst will be assigned to the case. They will oversee requesting the medical information necessary to process and decide your claim.

    In closing, I would let Social Security request the info from your doctor. They will let you know if more information is required.

    Thanks, Joe. Got confused as I already did the application online with cancer center social worker. One of the papers was the authorization to release my med records.

    The "packet" they sent me includes the med release form again, and asks for list of doctors,hospitals, etc. which was already included in online paperwork.

    Doing okay and yourself? My other doctor is concerned about my light-headedness and that my pulse almost doubles when I stand up, opposed to taking it sitting down. Soooo, that doctor referred me back to my general practioner who is ordering EEG, EKG, etc.

    Seems never ending.

    Kelli---what Joe indicates is most accurate.

    I applied June 5th, and was Just told a decision has been made and is in the mail and I will have it this week. So..process takes a couple months, I have heard much faster turnarounds for various reasons, but let the process work and inquiry occasionally on status via 1-800 number and claim number.

    Good wishes for a smooth process...

    Medical release Form
    Hi Kelli,

    I am doing ok. Beginning PCI Monday for 16 days at 2gy for a total dose of radiation at 32gy.

    Make sure you sign the release and bring it with you to the appointment w/Social Security. They will need it to begin requesting the information from your doctors. I do not know if you could use the Medical Release form that the hospital did with you as they may need a hard copy with actual signature.

    I just got a letter from SSA

    I already did these online with cancer center social worker...do I have to redo these? I have an app confirm number from then and when I look it up it says I will be contacted.

    I have tried to call their number and always on hold. How many apps are there????

    If your experience is anything like mine, that notice to complete the SSA app and the permission to release records is a form letter. Since you have completed the application online and have a confirmation number, all you really need to do is to complete the permission form (they require a signature so this can't be done online).

    It IS confusing, and I was lucky enough to get through to a representative locally who explained that to me. Confirm that with your social worker, but that is how it worked for me. Until they get that permission form, SSA cannot begin to contact your doctors to confirm your history and status.

    In my case, approval came without ever stepping foot in an SSA office, all via the web and snail mail, along with a couple of phone calls with my rep.

    I was sent by my DR last June after diagonise to get ssd he sent my report from the biopsi and I went in on Friday in person and was approved on Monday but you still have to wait 6 months to get your ssd checks started

    Well, SS sent me a request for more info on my work history, so I will fill out the necessary info and send it in, if approved by the end of August, that will make it a three month process. Yes Patricia you are correct on the six month wait for fisrts check.
