At another site that I check regularily, a heroic colon cancer survivor who has dealt with a lot has used this form to chart his treatments. He says he used this when he had to change Dr.'s. I loved the simplicity and pacing of it and he made it available to others.

    Thanks for posting this. I found it interesting and useful. I've had to write up a brief summary of my medical history a couple of times -- not an easy feat! This chart is a good idea.....and allows one to see very easily the relationships and timings between events.

    I was in a hospital lab getting ready for yet another test a couple of months ago and some dear young technician with a checklist naively asked me if I had any previous surgery. I didn't know whether to laugh or cry. I just said yes. Oh, can you tell me what? I asked her if she would like to sit down! Ha. Gotta laugh..

    I also laugh every time I am asked this question. Prior to August 2004 my answer was always only I had my tubes tied. Hospital stays? yea, with my two children's births. That was it! Now, like you said, would you like to take a potty break first? Mary
