pre employment physical and history

I went through colon cancer treatment/surgery over the past year or so, last surgery November/2004. During that process I also lost my job (downsized).
I am feeling great now, and wish to reenter the work force. I have received a decent offer but need to take a pre employment physical and history questionnaire.
What are my rights, what do I have to disclose, I still have my port in my chest, so it's not like they aren't going to find it.
Do I have a chance at getting this job?????
Any info would be most helpful.

I really don't know what your rights are. Do you still have a good rapor with your last job? My boss would hire me back in a heart beat. In fact, he sends me emails saying the company is
praying for me. It makes me feel great. Hold your head high and if a potential new employee asks what are your greatest accomplishments, you tell them proudly.. I faced the challenge of cancer and pulled through!! I think you would earn some respect and points here. Tell them that
you never gave up.. and you don't quit. They might hire you on the spot! Keep your chin up and go for a job you've always wanted! Best wishes and hugggs! ~ Wanda0 -
I don't know the legal ramifications either, but when I'm questioned in regards to my health, I tell them I'm a cancer survivor. I leave it at that. If they want to know more, I tell them, but I emphasize SURVIVOR. Good luck to you!0

I might call the Labor Relations Board for your state. And even your local library reference department should have info for you. With cancer so prevalent, there shuld be more of a safety net than we think.....hope so anyway.
