Transrectal vs Transperineal Biopsy
Is it worth waiting 6 weeks for a TP biopsy in the hospital, if a TR can be done sooner, in the office? (MRI Pirads 4, PSA 3.3, 71 yo).
Is Pirads 3 upgraded to 4 because of positive DCE the same risk as a regular Pirads 4?

Pirads 4 can also be benign. As far as waiting 6 weeks for the TP, it’s not unusual to have to wait 3 weeks for a TR biopsy, so the wait time should not make any difference if the TP is what you want for your own piece of mind. I know statistically the percentage’s are less chance of infection with a TP, but the percentage differences are not large. It’s often the overall health of the person going into the procedure that increases their risk. It’s really just what you are comfortable with . My own Urologist who has done hundreds of TR along with hundreds by the other urologists in his practice said their office has rarely seen them and mine none. He told me TR gets a bad rap primarily from infections they believe are encountered by the patient not strictly following after procedure protocol with strict before and after antibiotics. I had previous cancer and a compromised immune system. I even needed an IVIG infusion before the procedure and antibiotics for a week and the Urologist was totally confident I would be fine. He never told me not to get a TP one. Any procedure has a statistical risk of infection. Of course to anyone who has gotten one it is a serious matter and you will be more likely to hear from people on the site that may have have received one. Many patients have had multiple TR biopsies. I was in and out if his office in 20 minutes and basically painless.

TP biopsies are becoming more common place these days. I had a rectal biopsy in my Urologist office and had no issues with infection or discomfort, no anesthesia. I don’t think waiting 6 weeks is a big issue if you can only get the TP or your Urologist highly recommends it.

I've had both. TP Fusion targeted biopsy is the best in my opinion. Much more likely to find any cancer because they can probe accurately into areas of question that show on the MRI. TR Fusion with ultrasound guidance is good too, but there is a higher chance of infection and you'll need to take an antibiotic which in my experience messed up my intestinal track for a few weeks afterwards . I'm not sure if Random TR biopsies are still being performed, but they're clearly not as accurate which could lead to a higher percentage of false negatives. I'd stay away from those. An in hospital under anesthesia is a more pleasant procedure, but will take a bit more of your time. Overall these biopsies are relatively easy to endure. On rare occasions, a biopsy can cause urinary retention. Trust me. That is something you don't want to experience. My doctor prescribed flowmax for a week prior and a week after my last biopsy to reduce the risk of experiencing that again. Anyhow, waiting shouldn't make any difference. Pca is typically a very slow progression. Good luck with your decision.
