Prostate cancer with metastasis to nodes

Im 54 and recently diagnosed with prostate cancer and my PSMA shows 2 mesorectal lymph node involvement. I got next Wednesday for my first appointment with the medical oncologist. I've read a lot on treatments and very confusing. Some say I can't have a prostatectomy others say maybe with radiation. I'm just so confused anyone with any insight. I've also been told these lymph nodes are a rare area to go to.

    Hi,

    Yes wait till you’re finished with consultation with both your Urologist and Oncologist. Hopefully some of the confusion will go away. It not uncommon that once the cancer has left the Prostate surgery is not usually recommended. As far as I know it not that rare that the cancer spreads to the lymph nodes closest to the Prostate. Depending on the Gleason score(cancer aggressiveness) will play apart in your treatment choices. If cribriform was present that also could complicate things. If you had cribriform might be a question to ask your team as that could effect how you proceed. Again consult with your doctor team on how to attack the metastatic cancer. A second opinion might not be a bad idea.

    Hi,

    It is true that mesorectal lymph node "invasion" by PCa is rarely commented but this could be in cases of diagnosis not involving PSMA PET scans.

    Those patients probably wouldn't go through a treatment with a protocol aiming an attack to the lymph nodes in that region.

    That is not your case. The nodes have been identified by the PET and you should investigate further to have it included in the treatment.

    These could be handled together in one goal if you do radiation therapy or separately if you do surgery (dissect or RP +RT).

    In your next consultation you can inquire about having a biopsy of the nodes to certify that they are PCa metastasis which would define the best choice for your case.
