New to hear and have alt of concerns

  1. ADENOCARCINOMA; Gleason Score 7(4 + 3); Grade Grcup 3: 15% of the needle core tissue involved by tumor: 1/ of 1 core involved. (Confirmed by PIN4 IHC-G) |
  2. Benign seminal vesicle/ejaculatory duct tissue.
  3. ADENOCARCINOMA: Gleason Score 9(4 + 5); Grade Grcup 5: 90% of the needle core tissue involved by tumor; 1 of 1 core involved. Perineural invasion present.
  4. ADENOCARCINOMA; Gleason Score 8(4 + 4); Grade Group 4; 75% of the needle core tissue involved by tumor; 1 of 1 core involved.
  5. ADENOCARCINOMA; Gleason Score 8(4 + 4); Grade Group 4: 10% of the needle core tissue involved by tumor; 2 of 3 fragmented cores. Perineural invasion present.

  6. my urologist still won’t see me till the 23rd I know the Gleason score 8 isn’t good and the 9 is bad but how much worse is it with the perineural invasion is preasant in to places my 2 digest fears are 1 inconsistence and ed but not as much

    Hi,

    With your biopsy results I would hope that your doctor team would 1st schedule a PMSA PET scan to look for spread outside of the Prostate. With your Gleason scores the cancer is on the aggressive side. If the cancer is fully contained then you could do surgery or radiation. Perineurial invasion means the cancer is getting involved around the nerve bundles that surround the outside of the Prostate. I had PNI with my 3+4 10 years ago, I am still undetectable so all is not lost. If it was me, I would be on this ASAP, it sounds aggressive.

    Yes, you need to find out ASAP if the cancer has spread outside of the prostate. In 'the olden days' one would have a bone scan and a CT scan. These days, a PSMA scan is often prescribed because it is quite sensitive.

    What is your PSA, age and location?

    I realize that waiting is hard, but getting an appointment on 5/23 is reasonable under the circumstances.
