Need Urgent suggestion and help....PMP
- My father-in-law, J.N.Sharma, Age 58, had few episodes of vomiting, indigestion & weight loss until May'15 which was later diagnosed as a tumor in ascending colon with malignancy in the month of July'15.
- He was identified a case of PseudoMyxoma Peritonei/appendicular adenocarcinoma and underwent a surgery on 26th July at PGI, Chandigarh where following procedure was done: EL+Right Hemicolectomy+Peritonectomy+end ileostomy +DMF
- Post Surgery he was recommended for HIPEC, for which we consulted in Rajeev Gandhi Cancer Institute & found that it was not possible before patient recovers from current surgery. However, we were planned for IV chemotherapy after 4 to 6 weeks from surgery.
- He had started liquid & soft food intake & was recovering, but in 4th week after surgery, from 19th Aug he had repeated vomiting episodes without retaining any food or water. In PGi Chandigarh, it was diagnosed as small intestinal obstruction.
- He was put on IV fluids along with TPN for conservative management but didnt show improvement in one week. We shifted him again to Rajeev Gandhi Cancer institute and again he was put on IV fluids+ TPN.
- He underwent CT, Gastrograffin test which showed blockage & liquid around small intestine. Later tapping was done to reduce the liquid pressure, that didnt result any relief. Also, his endoscopy was done to find out possibility to put stent, however stenting was not done later.
- Its been 17 days in same situation & currently he is on IV fluids (DNS), TPN & antibiotics and there is no further improvement in removal of obstruction. On an average his RT output is 1200-1500 ml per/day, Urine output is around 1000-1200 ml/day & colostomy output(all liquid) is around 1600 ml/day.

It sounds as if your father-in-law is in the top hospital in India.
You have done a good job describing the clinical situation, but don't mention what the doctors have to say about his condition.
Given that he has so recently had surgery, it may be they are reluctant to do more at this time. Perhaps they are wanting the obstruction to resolve itself.
I am a bit confused about the liquid and its location. Was it inside or outside of the small intestine? If outside, perhaps they felt that there was enough pressure on the small intestine to reduce the diameter and contribute to the blockage.
Someone should meet with the doctors and discuss what their feelings are about the situation and how long they will wait to see results before considering other alternatives.
