Urolgist gave me some bad news, said removal of bladder and prostate needed to be done. REPORT of condition: INFILTRATIVE HIGH GRADE UROTHELIAL CARCINOMA.I have read this is a very common bladder cancer. At the age of 79 I question the outcome of this type of surgery. No C-T scans done as yet to determine the grade or stage--should this be done before a plan is put in place? Report listed-- CK7 positive within tumor cells --Ck20positive within tumor cells. P S A negitive within tumor cells.--P S A P same p63 positive within tumor cell nuclei. Tumor necrosis is present

From this report I have no idea of the stage I'm in or the degree in which the cancer has infiltered Can some one help me understand this report and treatment.

    Get second opinions from specialists

According to your previous thread, you have already started treatment for prostate cancer. Can you explain the reason for the above report? Did you get an extra biopsy to include the bladder?

“INFILTRATIVE HIGH GRADE” may refer to cancer that grows in the deeper layers of the bladder. I think it serious so that I recommend you to get second opinions from specialists. You may as well navigate in the CSN forum for bladder cancer patients.

Best wishes and luck in your journey.

    Prostate / bladder

Had bladder stones removed and a prostate (shaving) to allow the flow of urn.  did a biopsy of the bladder and the Dr. who did the procedure told wife " Laser did work, removed them by manual exstraction" Three days latter Dr. said " You have bladder cancer" End of conversation. Recomended me to a fellow Dr. in the same office and said he would be my Dr. They want to do a Di V. operation and remove both the bladder and prostate. There has been no C-T scans or MRI to describe the stage or grade. Chemo & Rads is another option but reoval seems to be the best option. I,m of thin build and quite active and ,  was in good health.  Had P E P scan done in Oct.2013 No Hot Spots  Thank you much for posting--have gone to bladder board.--- A thought , Prostate is still.  in its bed, -Bladder is probley a stage3 and my age is close to 80. Choice is fight the cancer and go through hell with the treatment or choice of Pallative care with chemo/or other chemicals and would need some kind of bottom line of time. None of us have a crystal ball to see but I am trying to make the best decision possible. I'm not a new guy at this --Had lung cancer and cancer of head  in 2010 &2011 Clean for over two years. Again thank you and all that post on this site

    Crystal ball

You are right; we have no “crystal ball” to see what is expecting us.

I am sorry for your cancer diagnosis. You did handle two of them and probably you will repeat the success again. In any case, surgery will harm your present quality of living. This is not good for the 80th when we start to feel old. I think that you should be confident with whatever treatment you are going to endure and so I would recommend you to only decide after getting second opinions from other specialists. I would collect all the information obtained from your present doctors and would pay a visit to a centre of excellence similar to UCLA or MSKCC.

Removal of the bladder and prostate in “one goal” is a major operation and needs to be addressed with positive thinking. Your family will be important in the decision process but they should be aware of the risks and effects from such a surgery.

I hope you find the best and the comfort you are looking for.

    2nd opinions

    Rarph,

If you're planning a move to Washington State soon as indicated in your previous thread titled "Treatment," then you owe it to yourself to obtain 2nd opinion consults and further imaging tests at the Seattle Cancer Care Alliance, part of Fred Hutchinson Cancer Research Center. It is a well respected medical/cancer tx institution.

Good luck with both your move and treatment decisions.

    The commenters are smart

Always, always get a 2nd opinion. And not from a doc in the same office/group.

    God bless,

    Richard
