Please provide some more info about your prostate cancer.

Very much in general, ADT is often recommended (along with radiation) when higher grade prostate cancer has been diagnosed. The length of ADT typically varies between 6 and 24 month, depending on the Gleason score.

Many years ago I had 18 months of Lupron therapy while undergoing radiation therapy (Gleason 9). So far, so good.

Been on Orgovyx along with Zytiga/Prednisone for about a year so far for 4+3 metastatic PCa to the pelvic lymph nodes. My Fox Chase Cancer Center oncologists recommend a total of 2 years. Like you, I started it a bit prior to IMRT. This treatment regimen has worked very well for me, now having an undetectable PSA for 9 months down from a PSA of 9 prior to treatment. This drug regimen has been tolerable but has it's challenges. I feel fairly well, but suffer a bit from fatigue, hot flashes, and not sleeping well. My oncologist has agreed I should stay on Orgovyx but to stop taking Zytiga and replace it with Nubeqa (Darolutamide) which has reportedly less side effects than Zytiga and potentially higher efficacy. Nubeqa also doesn't require Prednisone which is a big plus and I believe was keeping me from sleeping well. Anyhow, that's just my experience so far. Everyone's experience can vary. At this point in time, I believe Orgovyx to be the superior choice for ADT due to it's a daily oral medication (no shots). It can be stopped easily if needed and has a much faster testosterone recovery when compared to the injectables.

Diagnosed Dec 2024, Gleason 8 with mets to local lymph nodes. They classified in stage 4a. Did a month of radiation last May and just had my 3d Lupron shot. Taking zytiga daily along with prednisone also. The side effects have gotten worse over the last several months. In addition to the usual suspects (hot flashes, fatigue, of course total loss of any sexual function) it has started to impact me mentally. Brain fog, anxiety, some depression. MO wants me to stay on the ADT for another year. At least the PSA continues to be undetectable, but yeah the side effects suck. Oh, and my insurance won't pay for Orgovyx.

I've been on Orgovyx ( outrageously expensive ) for about a year and a half plus Nubeqa. My PSA has been practically non-existent. Except for weakness and constant hot 🔥 flashes it's been tolerable.
