PSA 4.5 after IRE

i am shocked that my psa is 4.5 three months after IRE procedure. I did have the blood drawn the same day I had sex- not knowing ejaculation could alter the results. But 4.5 is very high…not sure what is next.

    Hi,

    I would wait for a few weeks and retest with no sex or bicycle riding before your test. Both activities can elevate your PSA test results. What was your PSA before your IRE? Your PSA will bounce around some due to the live Prostate tissue producing PSA.

    Sounds like sound advice. PSA was 5.0 at the time of the procedure.

    Can you be a bit more specific about your IRE procedure?

    For instance, how many lesions were treated and what was their Gleason score?

    PS: The success rate of the Nanoknife procedure depends on the Gleason score of the lesions treated.

    I too was concerned about a one point drop in PSA from 5.6 to only 4.6 three months after my IRA procedure. At that time my Fox Chase oncologist who performed the procedure didn't seem too worried and ordered another PSA test in three months which came in at 9.3 which did worry him. An MRI and a transperineal biopsy were performed both showing all clear. So next a PSMA PET scan which unfortunately showed metastasis to a couple lymph nodes in pelvic region. I was then started on ADT and have had 40 sessions of IMRT. Now I'm just continuing the ADT with PSA tests every 3 months. My PSA is now in the undetectable range of .06 which has risen slightly from .04 on a prior test. Doctors tell me not to worry but we'll keep tracking it. Yeah right! No worries. Lols!

    Anyhow, if your PSA continues to rise I imagine you'll have some imaging done likely MRI and PSMA PET and perhaps another biopsy? On a more positive note, it's only been three months since your procedure and being that you still have a prostate the inflammation from the procedure could be causing the slow drop in PSA. You might want to ask for another PSA in a month. If there is any metastasis, the sooner they know about it the better. I wish you all the best luck on a continuing drop in PSA.
