Sbrt question about how low psa dropped
Finished up my 5 doses end of Dec. I was wondering how low did your psa drop to? I was psa 8.5 Gleason 4+3 initially. Went on orgovyx 6 weeks prior to radiation, 4 months total. At last treatment session, psa was 3.8. Next blood work beginning of May. Was hoping for some real life statistics. Thanks in advance guys.

Hi,
Your PSA should be stable over several months/years which will indicate your cancer is in remission. Since you still have live Prostate cells each persons reading will be different depending on how much Prostate tissue was destroyed by the radiation treatments. Stability in readings is the key.

I finished SBRT in December 2023, and was on ADT for 6 months until June 2024. I was 4+3, high Decipher, pre=treatment PSA of 7.8. I had a big drop in PSA post SBRT and while still on Orgovyx. It was been relatively stable lately. It can take 24-36+ months to reach a post ADT nadir (even though the ADT induced nadir is considered THE nadir). My RO is satisfied with my results at this point.

If I 'calculate' this correctly, you were on Orgovyx until the end of February, roughly. Testosterone recovery is supposed to be pretty fast with this drug, but you should ask for a testosterone test to be included the next time you visit the lab (May).
I don't like to bring bad news, but I would have expected your PSA to be lower at this stage. I had SBRT combined with IMRT (Gleason 4+5). Prior to therapy, my PSA had risen to 12 ng/ml. It dropped to very low (0.1 ng/ml) while on Lupron and, when that wore off, went up to about 1.4 ng/ml. The last two years it has gone down a bit to about 1.0. Very happy with those results, obviously.
With respect to your case, a repeat PSA test seems warranted right now.

Thanks. I have had my testosterone tested several times since I went off Orgovyx and it has returned to 90% of the level prior to starting ADT.
I am confused that you are concerned with my PSA of 0.65, while you think your level of 1.0 is acceptable. I assume you are not a physician or radiation oncologist, and while I appreciate your opinion, I'm going to go with what my radiation oncologist says.
