79 year old with Advanced PC to L-1 & Pelvic

I Went to ER with Urinary Retention on this past December, 2024. Inserted Cathether due to Very large Gland
First Urologist visit confirmed PC, CT couple of weeks later confirmed APC
Then PSA with a high Gleason of 4+5.. Had first HT Firmagon injections a month ago, 2nd injections coming up in a few days. Speciality Cancer Drug NUBEQA is pending for financial assistance. Cathether still inserted…I still am considered a healthy athlete for my age, so am considered a good canditate for the TURP surgery to shrink the gland.
I understand that everyone's system is different with the endurance of PC treatment..
Unfortunately, I feel personally that I am in the info Grey area with My Care Team just being a name and birthdate.
Am I on the right path for treatment so far?

Hi,
Do you have a little more info on your test results, ie; position of the tumor inside your Prostate, any spread outside of the Prostate, how many sample cores were positive and associated Gleason score? A score of 4+5 is pretty aggressive. A Turp is usually done to widen the urethra area and should do nothing to reduce or put your cancer in remission. Hopefully you are considering some type of treatment for your Prostate cancer. Usually at 79 a better course of action might be some form of radiation treatment.

No, when I mentioned My info is not completely an open book on My patient portal, been told from practice that they are working on a new system. So my notes will be asking your very questions. It has methasized to My L-1 & Pelvic bone confirmed by CT in January. Hence: First Firmagon HT injections. (Testerone Castration) last month, 2nd next week with prior updated Blood tests.Pet CT tomorrow
Yes, understand the Turp and its purpose, kind of a double whammy fighting that separate problem

Thank You for replying .Understand, your info on the HoLEP over the Turp. Unfortunately the area I am in is controlled by one Group of Urologists and Ongologists. No Independent doctors. Yes, my research has said that is the gold standard for hallowing, which they don't offer. I agree with the research on that procedure, alot of Ongologists aren't trained to do HoLep over Turp.
