Hello all, new to the forum. 66 year old male. Currently awaiting fusion guided transperineal prostate biopsy (next month). History of elevated and increasing PSA (last 3.8) but relatively normal sized prostate (31 cc). Just had multiparametric mri (3t) and results indicate a single pi rads 4 lesion (12 mm) in right mid anterior peripheral zone. My symptoms include trouble urinating (hesitancy, low flow, etc) and pain in lower pelvic areas (bladder, perineum, testicles, penis). Pain is worse at night (early morning). Wakes me up 2-4 AM most nights. I get some relief after urinating. Pain would cease as the day progressed but now is ever present. I've had a bladder cystoscopy and it "appears normal" - no cancer).

I feel confident I'm on the right diagnostic track but in the meantime fairly miserable dealing with the symptoms. The urologist (understandably) is focused on a cancer (yes/no) diagnosis and has suggested I see a pelvic floor PT specialist for the pain. I have an appointment with a clinic but I'm concerned the pain's origins lies elsewhere (advanced cancer?, prostatitis?).

Does anyone here have or have had symptoms as I've described? If so what was your outcome?
