Need help read my Path report

Can anyone explain this for me. I just got my pathology report today. My PCP said I am 100% good. I just want to get our survivors opinion.

Tumor size: 4.6 x 4.5x 3.5 cm

Focality: single gross lesion

Limited to the kidney/ clear cell

Grade 4

Pathologic TNM Classification: pT1b, pNX

Stage grouping: Stage 1

Sarcomatoid feature: Present

Tumor Necrosis: Present

    Hi

    i am sorry but you have a mixed Report

    the size is good as it was no doubt completely remved which means now you have no more cancer

    However the sarcomatoid and necrosis means your chance of recurrence is hgher

    please dont go into panic mode but you will need very close monitoring  to ensure that if something  does pop up it can be treated ASAP

     

    Please get a schedule set up that you are happy with

    I'd go see a medical oncologist with experience with RCC to get a prognosis and followup plan. Grade 4 and those other features are features of aggressive RCC. You need a knowledgeable opinion for a decision on your followup care. If I were you, I'd want baseline scans as well just to make sure you don't have any tumors anywhere else at the moment that happen to be symptom free. The goal in your followup care is to catch recurrence as early as possible while it is still operable. The later you catch recurrence, the more likely it won't be operable.

    You don't want a family doc, a urologist, or even a medical oncologist that isn't familiar with RCC making this decision. This is a decision better suited to a medical oncologist with lots of RCC experience. If you can't get that, try and find a medical oncologist with a related specialty (say melanoma, which has some features in common with RCC) that is willing to maybe consult with an RCC specialist.

    By the way, there's nothing in your pathology report that indicated a diagnosis of RCC and type of RCC. Was that somewhere else? You also need to know what type of RCC you have. Most of us here have clear cell, but there are several other types. Those other features you have might indicate that it's not clear cell RCC. If you have a rarer type of RCC, that's an even stronger reason to see a specialist familiar with RCC.

    If you scroll down and read "Fuhrman Grade is VERY IMPORTANT", you will better understand my reply. The Fuhrman grade and those other features present are all indicators of aggressiveness. Have a read of that thread. It might help you understand your pathology report.

    Fat overlying tumor ( Excision)

    - Benign adipose tissue

     

    Deep margin (Excision biopsy)

    -Negative for Atypia

     

    Left kidney (Partial Nephrectomy)

    -Clear Cell Renal Carcinoma, Fuhrman grade 4

    Margins:

    Ureteric:   N/A

    Vascular:  N/A

    Parenchymal: Clear

    Soft tissue: Clear
