pain and lump in nipple 2yrs after nipplesparing mast

Has anyone had a nipplesparing mastectomy? In 2014 I was dx with dcis/lcis and chose to have a mast. Since no cancer cells showed up in the nipple during surgery, the surgeon left my nipple during reconstruction. 7 mos. Ago I began having acne like bumps and discoloration under my nipple so I started using cortisone 10. The bumps would come and go. Shortly after, I started having a dull ache off and on behind my implant and about 2 mos.ago, my nipple began  to hurt to touch and I noticed a lump in the areola. Unfortunately, the Breast Surgeon won't see me since I've had a dbl mast and the earliest I could get an app for the PS is 1 month. So, I started massaging the lump in case it's scar tissue. My husband thinks I shouldn't. I thought the lumps and bumps would be over since I had a dbl mast. This is so nerve recking! Anybody ever have a reoccurrence after being dx with dcis /lcis clear margins, no nodes, and mast?
