How do you file claim with insurance for adjustable bed

I am looking for adjustable bed much needed after surgery. I wonder if someone knows how to file claim with insurance company. What code to file for adjustable bed.  Greatly appreciate the help if someone knows

    In my husband's case, when an

    In my husband's case, when an adjustable hospital bed was needed after surgery, the attending doctor simply wrote a prescription for the bed. That is the start.  Then it depends on what , or how much the insurance company will cover, and if you have a co-pay. I asked the physician to prescribe  for a bed side tray with wheels and a hospital type easy chair with wheels and that too was provided. They were ocourse rentals and returned when he no longer needed tham. I hope you are successful with the insurance company and will need it only short erm. Blessings.
