Primary Tumor (T)
T1c Tumor identified by needle biopsy (eg, because of elevated PSA)
Tumor found in one or both lobes by needle biopsy, but not palpable or reliably visible by imaging, is classified as T1c

Regional Lymph nodes (N)
NX Regional lymph nodes cannot be assessed

Metastases (M)
MX Distant metastasis cannot be assessed

Grade / Pattern
Grade 3 + Grade 3 = Score 6
Grade 3: Cancer glands vary considerably in shape and size and they spread into the surrounding normal prostate.
This is the most commonly seen grade.

Diagnosis
Prostate Needle Biopsies:

A. Right Apex Medial: ADENOCARCINOMA (Gleason 3+3=6) involving 5% of the specimen
B. Right Apex Lateral: Benign prostatic tissue. Chronic inflammation.
C. Right Mid Medial: Benign prostatic tissue. Chronic inflammation.
D. Right Mid Lateral: Benign prostatic tissue. Chronic inflammation.
E. Right Base Medial: Benign prostatic tissue. Chronic inflammation.
F. Right Base Lateral: Benign prostatic tissue.
G. Left Apex Medial: Benign prostatic tissue.
H. Left Apex Lateral: High-grade prostatic intraepithelial neoplasia.
I. Left Mid Medial: Benign prostatic tissue. Acute and chronic inflammation.
J. Left Mid Lateral: Benign prostatic tissue. Acute and chronic inflammation.
K. Left Base Medial: Benign prostatic tissue.
L. Left Base Lateral: Benign prostatic tissue.

Notes: An immunostain for PIN-4 (high molecular weight cytokeratin, p63, Racemase cocktail) was performed on specimen A. The findings support the diagnosis.
