Hi all

What i,m looking for is a printable medical form that show,s what Meds i am taking and a grid for each day so I can write down when ive taken the pill (I seem to be forgetting the times now )

It would be something like in a PDF file so i can add the times on the computer then print it all out our save it to a flash drive to take to my doctors for her to look at

I have the blood pressure cuff and the O2 sensor that i can plug in and make that chart out so want to make the other
