Or having surgery / Chemo / Getting a prescription, etc., this list is a pretty good resource! I'll be adding the address to my page.

The Dept. of Health & Human Services have a great list that can be the starting point for the questions you should ask your doctor. The second page has even more detail and more printable lists.
