Hair donation requirements, PLEASE HELP!!

[I'm going to try to make this as short and to the point as possible]
About 3 years ago, I lost my uncle who was like a father to me. He was diagnosed in September of 2007 but lost his battle and passed away 10 months later. So through this difficult experience, I have decided to do my best to help others who have and are battling cancer. One of the several ways that I've decided is to grow my hair and donate as much as possible every 2 years.
- Who knows the length requirements for the American Cancer Society and where I can go that is close to Salt Lake City, Utah?
