Patients rights - requesting a move to another hospital

I am beyond frustrated with the hospital my mom has been in for the past month. Has anyone ever requested their loved one be moved to another facility. This wouldn't be in the same network.

The nursing/doctor care has been less than informative, supportive and helpful and we have one of the top hospital just down the road.

What is the process for moving a patient. I assume it would be our responsibility to get her there?
