I know that there are so many other things going through your mind when you go through diagnosis, treatments, and even follow-ups.

But one thing that gets forgotten about, and while not may be that big of an issue in the digital age, but for those who still deal with paper records, it is so important that you keep a record at home of everything you (or your family member) are going through.

As I found out the hard way, most of my records had been destroyed. And this past year, when I was introduced to late effects of my treatments, the doctors that I would be seeing had requested them. While not making it impossible to see me, having the records would have made it easier to have dosages, frequency and such.

Just an FYI and reminder that being part of your treatment team, which you should be, means having record and knowledge of what you're going through.
