So many choices available.

Looking for feedback.  71 y/o/ man

Pros removed 2015. PSA 23. Gleason 7 .  Some lymph removed too.

All good for about 3 years. PSA started to slowly climb.

As of 2021, it was .975

Received hormone shot April 2021, latest PSA .027

I have narrowed it down to  Xray, Proton therapy or "watch and wait".

I'm more interested in quality of life than quantity.

I'd love to hear your opinions!

Hi there,

If your doctors think that the cancer is restricted to in and around the prostate bed I would go for six months of ADT, hormone shots, plus either radiation or proton.
That has a good chance of knocking it on the head fairly quickly, you will only have six months of ADT, so you have a moderately good chance of recovering some testosterone with its benefits as well, although it has to be said that at 71 your age is against you.

Hi,

Do you know where your cancer is hiding inside of your body?  If it was me I would want to know what I am dealing with via MRI or PET scans then detemine the treatment.  Once the cancer is located then any of the radiation type treaments, Cyberknife,Proton, ect. would be a good choice to kill the bandit.  ADT will deprive the cancer of its "fuel" for a while but the cancer will learn to grow without the testosterone.

Hi there,

I doubt that scans would find much.
He started with a low PSA and he's opted for ADT. The result of these factors would be that any hotspot will be lying doggo.
He had a starting Gleason Score of 7 which would tend to indicate that any spread is close by.
It might be worth a punt if he can persuade the docs to authorise a scan, but I doubt they will play ball.

If you can't find it you can't kill it unless your talking chemo. Could be in his bladder, kidneys,spine, prostate bed, ect..  Proton and Cyberknife are pinpoint technologies that don't do much for shooting in the dark.
