We are still in ICU. Bili numbers continue to climb is at a 19 now. They are preparing to go in and scope the bile duct tomorrow probably since they don't have a team in today. He has been running around 102 temp indicating possible infection. He has fluid in his lower right lobe of his lung so are treating for pneumonia even though they really don't know if he has it but feel it is better to play it safe. Still having moments of confusion, not tracking with his eyes, when they hold up 1 finger he thinks it is 3, very yellow eyes and skin. His blood still needs to come down to 1.5 before they can pull the epidural. He thinks it is out and insists we don't know what we are talking about and wants the mri done and over with. can't do the mri until the epi is out and so just a chain effect. Regardless by Tuesday they have to pull the epidural out and we deal with any consequences. So today they are really pushing the Vit K, more plasma and something else. Renee |

I don't unless you count the dogs
his goddaughter lives 30mins away and has been coming to check once a day and bringing things if I need it. Since the original plan was to stay for surgery and leave after he was settled in a day or two later he thought having the dogs with us while he was in surgery would be a good thing to keep my occupied... well they have turned into a source of forcing me out of the hospital to go check on them every few hours thus meaning fresh air, stretching my legs etc so its a pain but also good to have. If I need his Goddaughter here all I have to do is call her.

Keep em coming he needs everything
Got excited his INR levels came to 1.3 so they swiftly took out the epidural when they saw it was in range. not going good here otherwise right now they had to back out of the scoping his bp became very unstable and the more they went in to try and do the procedure his bp finally dropped to 60 and his oxygen levels desaturated and they had to pull out. So guess nothing else will happen today. He has become very uncontrollable as it was it took 45mins for me to get him under control they were threatening to put a straight jacket on him as he kept trying to punch everyone insisting they were trying to kill him. Everyone was forced out of the room in order for him to relax then a nurse snuck back in to give him something for pain which made him sleepy. I covered him up and he started talking again and said I am dead I don't need this blanket you keep it ..does me no good. He has had 4 other mini short lived episodes but has been sleeping now for about 1 1/2 hours. Kept insisting I needed to take him outside so he could see he was still alive. I told him he wasn't stable so we couldn't do that. he tells me to get a wheelchair that will help but he needs out of here because they lied to me and are really trying to kill him.
I pray he doesn't remember anything from this week after this is all over. Sadly it will be with me a very long time. For now he rests.

I had the same reaction to whatever damned pain killer they
were giving me. I wasn't like that anytime after my first operation
in 2006, but this last one in 2011 was impossible.
I had hallucinated things that seemed real to me, and attempted
to crush a nurse's hand. I thought everyone, wife included, was
conspiring to kill me. I had wrist and ankle shackles applied,
an NG tube and a respirator assembly down my throat.....
The nurses said that they saw that insane reaction very often "lately",
but didn't speculate what drug was doing it.... So don't worry
about his reactions, moods, etc., they are not his normal and
have nothing to do with the "real him". He needs comfort and
reassurance that it's the drugs that are making him feel the way
he's feeling... It's tough, but try to get that across to him.
And if you can, tell the physicians responsible for his meds,
to please find one that won't cause anxiety and paranoia.
There's no reason that reaction should be happening to so many
patients; the nurses see it, the patients experience it, and the
entire care giving population has to endure it.... It's time to fix it.
My best wishes for him, and you!

What John23 said and then some
One of my surgeries involved Dialuid (not sure if that's the correct spelling), which is about 10x stronger than morphine. It was not good. At all. I went into another world that seemed very very real, and very very scary.
Please talk to the Dr. about this...it is not uncommon, but very scary for everyone involved. There are other pain meds.
Good luck~

That's good advice to check
That's good advice to check on the meds and to know these behaviors really aren't him. Whether it's the pain meds or other meds, something else is making him this way.
I guess having the dogs can be a good thing since they get you moving and getting some fresh air, a breather from the intensity. I just wanted you to know I'm keeping you and Carl in the light and in my prayers.
Bless you for your vigilance.

thank you
Thank you to everyone for the advice and prayers, making sure I think to eat etc. They put in a pic line earlier this afternoon and did an ultrasound and when doing that they found a huge fluid pocket in place of where the liver use to be. So getting the IR team in (hard to do on a holiday) so they can drain it and they are hoping it has been the cause of much if not all the problems we have been having from fevers to the passing out, jaundice etc. Will keep posted. I want to start jumping up and down with excitement that maybe he will finally be on the road to recovery but I am hesitant to do so just yet.

Today they just wanted to get the bile that was accumulating out and they were only able to leave one drainage tube not the 2nd that is why another procedure tomorrow to redirect things in the right direction

got better details
Okay his regular dr. explained better. What has happened is the side wall of the bile duct was weak, got nicked or who knows exactly but the bile takes the path of least resistance and regardless he has a leak that needs repaired. So todays procedure is to go back in and put a stint in there and hopefully correct the situation. Last night they just wanted to get him drained from all the build up. His bili went up to 23 and is down to 21 this morning so we are in the right direction I think.
Sadly when they tried to scope the first time he probably really did think they were trying to kill him....he felt everything they did under the opiates they used. So when they go in today it will be under general anesthesia. I feel we are in the right direction. He is still really short but seems more clear headed so I am feeling better.

so sick of the yoyo
still hanging on to any positive thought of hope. They got the labs back and the gunk they pulled out was not bile as they thought. meaning the leak is not the cause of his high bili numbers. They are going to proceed to try and put the stint in today they don't want to do it via a surgery but will reopen him if needed. They are using General An. this this time for sure. His INR is 1.5 so we will proceed soon probably this afternoon. They have said they hope it is a leak, but I needed to be prepared that this is more on the lines of liver failure. I am holding on to anything but that. I don't know how things go at that point or options, if there is any etc and I simply can't think that way. Depending on how this surgery goes I am having the kids brought down from MT

I thought I saw a terrorist when I was on pain killers, turned out it was a metal trashcan but it took some doing to convince me of that, so I would pretty much blame his confusion on the pain killers, we now laugh at some of the things "I saw" on pain killers, they were pretty amazing.

funny you should mention squirrels
In middle of Denver I saw 3 of them yesterday on my walk around the outside of the hospital. I firmly believe animals have messages for us. Sadly Squirrel comes with various from one end to another. From Warning to perseverance. I have 3 kids so wonder if seeing 3 different squirrels in short frame time is warning for them to be here maybe? I don't know how to read this one. Or to message to not give up and keep moving forward.

his bili numbers from this mornings labs are 22 so on the rise still, phosphorus is 6 which is higher than they want, glucose shot up to 400, temps are still 101-103, Got stents in yesterday afternoon-2 of them they found 2 leaks, he had a bm last night first one since morning of surgery. He was delusional again coming out he didn't even respond to me so restraints became necessary. that was incredibly hard to watch. He said he wants to go home so bringing home to him and kids should be here later today. Waiting for surgeon to make his rounds give his thoughts. Before when I asked about whether the kids should be brought was told lets not make any decisions yet, this morning let him know my plans to have them brought down and he said he would have a counselor come see me to help arrange hotel, when they see him for the first time etc. Not really sure how to take that, but in many ways I could use them here for me too. That is what I have for now. Waiting on retake of labs and see what they say. Dr. said he has his suspicions of what is going on but wants to wait and see what the new results say.

wait and watch
Today his bili went up to 24, he wasn't hungry but the dr said if he would drink protein shakes today he wouldn't put in a feeding tube. He said he would so here is to hoping he did. I have stayed at the hotel with the kids the afternoon for some sleep. I am still feeling loopy. Was 1.3 day before surgery, day of 3.6 and just keeps rising.
They are moving him out of icu as there really isn't anything further they can do for him but watch and hope the liver is viable and kicks in. He will definitely be there another week. His liver has plenty of blood flow so that isn't the issue, so far the stents in to stop the leaks are stopping the bilis from climbing. The dr. this morning just looked at me and asked if I looked this morning numbers I said yes and he just said I don't need to say anymore. That almost is harder than just saying something. I am still hopeful the remaining liver is good enough to work. Maybe all the meds to control pain has kept it from doing its job so far. They took him off IV psa button and put him on oral. He is still having problems with is right arm. They aren't saying he had a stroke but a definite seizure. I still can't help but wonder about a mild stroke. Seizure is more consistent however with liver trauma. The liver has good blood flow I am holding on to that. they did find a blood clot so giving heprin via iv rather than belly.

Praying things turn around. Good blood flow, taking in protein, and having the pca discontinued, stents working,.....should help.
