METASTATIC BREAST CANCER & CYSPLATIN+GEMCITABINE CHEMOTHERAPY

Let me tell you about myself, my name is Abhishek Sharma & I live in New Delhi (INDIA). I was going through your the website for the latest information on Cancer Treatment / Alternate Therapies etc.



Below is the short & Crisp Treatment record of my wife:-


My wife Ritu is been suffering from Breast Cancer from 2011. Inittialy she was diagnosed as Infiltrated Ductal Carcinoma Stage 3B, with ER-NEG, PR-NEG, HER 2NU-NEG.

 

As of now she is on the second line treatment of GEMCITABIN + VENORELABIN. The schedule of her Chemo is- Day1 then on day8 & then Day15. Our Oncologist is now planning to add CARBOPLATIN / CISPLATIN from coming 29th June & the same has been added now she has take 2 cycles of VINORELABIN - 80MG + CYSPLATIN 100MG ON DAY 1 & ONLY VINORELABIN 80MG ON DAY 8. After completing 2 schedules she is now suffering from High Fever, Low WBC (500 & 1100 on 29th July.) Our Doctor is admitted her in the hospital as he told us that he wants to isolate patient from the other people coz this period of treatment will attract infections. Now she is stable today. We have already added lot of semi solid foods & juices to her diet schedule and trying that she takes a smaller amount after every hour or two.

 

Rest of the records are attached, I request you to please see the case and put your suitable advise on the same.

 

 

HISTORY OF TREATMENT

 

In Sept - 2011 she has been diagnosed as a case of "Infiltrated Duct Carcinoma ca Left Breast - PT4b N3/10 Mo -Stage 3b & we have admitted her in Lady Hardinge Hospital New Delhi because one of our contacts was Professor of Surgery there. Under the expert supervision of Surgery Head Dr. Manoj Andley & Dr. Ajay Kumar. They have started CAF based Chemotherapy after all the required investigations like : Mamorgaphy, Bone Scan, USG, X Rays, CBC etc. After 3 cycles of CAF based Chemo they done Masectomy & removed the breast after this they have given 3 more cycles of CAF based Chemo. After completion of this they have advised us for Radiotherapy & she has take 25 sittings of Radiotherapy in AIIMS. After this the Doctors of Lady Hardinge Hospital advised us to visit in OPD after every 15 day to keep a vigil eye on patient. We have done exactly same which they advised.

ER- NEG
PR- NEG
HER 2 -  NEG

Now just before 2 months from now like in October 2012 she has seen a little swelling on the Mastectomy site with pus discharged. We visited to or Doctor once again & they have started some antibiotics like Augumentin 625mg & Cefdinir but the problem was still same then they advised FNAC & the report was negative. After some days we have observed a small nodule on left upper arm & we have done a biopsy for that & the result was same that the nodule is infectious with Cance PT4b N3/10 Mor. After that Doctors advised a PET CT SCAN.

The report was:-
Left Breast : Recurrent Disease
Right Breast:
A small Hypermetabolic Nodular Lesion in Right Breast (Inner Upper Quadrant) - Metastic Lesion in Contralateral Breast with Lymph Nodes (Bilateral Internal Mammary Right Axillary Prevascular Right Paratracheal & Left Cerical) Metastases.

No other Hypermatabolic Lesion in rest of the body.

They have started now Dosetoxil based Chemo & now on 12th FEB/13 the 5th cycles is done Chemo done & site of Surgery is very much swelled & the disease is little advance now.

 

After taking 5 cycles of Dosetoxil 160mg there was a little response till 3rd cycle and when 5th cycle finished doctor advised for PET CT & report shows MILD PROGRESSION in disease (recent report is attached). Also we have done ER PR & HER2NU 10 days back (report attached), here ER – Positive (0.05) rest are negative.

 

 

Now on 12th March Doctor started new Chemotherapy with new schedule as below:-

 

Drugs:- Vinorelabin 50mg + Gemcitabin 2gm.

Treatment cycle is:- day1 – then day 8 & then day 15

I need your expert advice on this case & need your appointment as soon as possible.

As of now she has taken 4th cycles of (Vinorelabin + Gemcitabine) & 22 June start 5th cycle.

 

 AS ON DATE 17TH AUG/2013-Most latest - Our Oncologist is started CYSPLATIN 80MG + VINORELABIN 50MG. TILL TODAY SHE HAS RECIEVED CYCLE 2 DAY1 TREATMENT.

DOCTOR GIVES HER - DAY1 - CYSPLATIN 80MG + VINORELABIN 50MG AFTER THAT DAY8 ONLY VINORELABIN. BEFORE STARTS CHEMO HE GIVES HER THE INJECTIONS OF NEUPUGEN 1 DAY BEFORE CHEMO & 1 DAY AFTER CHEMO.

 

SOME MAJOR SIDE EFFTECTS ARE OBSERVED BUT AFTER 2/3 DAYS IT DISAPPEAR. 

 

IF ANY BODY COULD ADVISE ME ANY HOME MADE OR ANY OTHER THERAPY WHICH IS FRUITFUL FOR HER, I WILL BE DEEPLY OBLIDGED FOR EVERYBODY.

    So Very sorry...

    Dear Abhishek:

    I am very sorry that your wife has to be dealing with all this chemotherapy.  I had chemotherapy back in 1994/95 and now on Taxol.  I haven't any experience to share with the kind of chemotherapy your wife is using.  No one on this discussion board are doctors.  I suggest that you talk with your wife's oncologist.  I haven't done alternative treatments and don't know much about them.
