Replacing aging/ruptured implants

Has anyone undergone surgery to replace aging or ruptured implants?
If so, did you replace them with implants again or did you have a DIEP/SIEA/GAP fatty tissue transfer reconstruction?
Did you have your implants removed "en bloc" -- inside the scar capsule? Did you have any loose silicone in your body that needed clean-up?
How was your surgery and recovery the 2nd time?
