in Ovarian Cancer #1

Well my mom who is 83 just finished her 3 rounds of taxol.  At the end of each of the rounds cr 125 scores were  62 t0 59 to 66 to 77 and I am expecting this last test to be at 90 or more.  Prior to the taxol she was on taxol avastin her test scores were 310 to132 to 77 to 62 then she had a seizure and went in to the hospital.  Wondering if I should suggest the avastin again the Dr. is looking to chage off of taxol to something different but seems avastin looks like avastin is the drug to choose

    It is hard to speculate without knowing your mom's history and counter-indications and I may be way off base here.

    I am assuming that she is platinum-resistant (her cancer recurred in less than 6 months after her first line of chemo Taxol+Carboplatin). She was put on Taxol+Avastin and that combo was working; but Taxol without Avastin is not.

    The best thing would be to get genomic blueprint of her tumor (it's free through Clarity Foundation). Instead of playing hit and miss with different chemos and accumulating irreversible side effects - they will test tumor cells in vitro and tell you the best individual chemo choices for her.

    If that is not an option, next chemo to try is usually the doctor's choice based on their experience and insurance requirements. Sometimes they leave the strongest chemo for last, when everything else will stop working.

    CHEMO: Cyclophosphamide, Docetaxel, Etoposide, Gemcitabine, Liposomal Doxorubicin (Caelyx), Melphalan 3, Paclitaxel (Usually used with carboplatin if cancer recurs one year post treatment), Topotecan, Vinorelbine

    anti- VEGF anti-body: bevacizumab (Avastin); PARP inhibitors (still in trials): Olaparib, Veliparib, Niraparib, etc.; Hormonal agents: Tamoxifen
