For those of you who had lumpectomies - anyone have wire localization done the day before?

One more question for my friend who's getting ready for her lumpectomy in a couple of weeks: She goes to the hospital the day before her surgery for the sentinel node biopsy dye injection (I know some of you did this as well) -- but she also thinks they'll do the wire localization the day before, too. And then send her home for the night.

I can't believe they would send her home with a wire stuck in her breast? I remember, for me, the lidocaine was starting to wear off as I was waiting to be wheeled into surgery, and the pain was starting to get pretty bad -- I can't imagine going all night?

Anyone have any experience with having the wire localization done the day before?

    No experience
Sorry, no experience with wire localisation done the day before.

I had my wire localisation (with local lidocaine) done on the morning of the surgery (ca. 9 am), as well as my radio-isotope injection (ca. 10 am) and then the blue dye injection (during surgery). Surgery was around 2 pm.

I agree that wire localisation the day before sounds uncomfortable for the following night.

Same here, wire locator procedure was performed in the
hospital 2 hours prior to my lumpectomy surgery. A small plastic 2 oz cup place on top of locator's to eliminate any contact to my surgical grown. Again, this was approx. 2 years ago, so maybe procedure and device have been modified.

    Just my experience.

    Wire Localization
I had a right mastectomy in Feb. 2010. The MORNING OF my surgery I went to the hospital's breast center and had the wire localization done. I don't remember it as being painful just strangely uncomfortable. The breast center was actually down the street from the hospital so my husband drove me to the hospital after the wire was in place. While I guess it would certainly be possible to do the wire localization the day before, I do not think that would be standard procedure. Seems to me that there would be a risk of infection and also a risk of the wire being displaced if left in for any length of time. Hope your friend gets this all straightened out. IRENE

Hmmmmm...I truly think I remember EVERYTHING that happened to me from my diganosis until today, but I don't recall having the wire procedure done AT ALL! It seems to me it wasn't done, beucause I don't even know what it is, or what its purpose is...looks like I have to do a bit o Googing! And, I see my oncologist on Tuesday~ I will ask him if this was done for me!

    Had both...
My lumpectomy ( two yrs ago this past week, yeah!) was done at 1:00 went at 10:30 to my radiologist, who does my mammos, she did the wire needle localization....I went straight to the day surgery center across the street....the dye was injected 4 times around my nipple...they numbed each area with a spray before each injection....felt like a bee sting each time....I can't imagine having all this done the day before....I had to wear, per the radiologists' instructions, a large button down shirt, nothing that went over my head....easy on and off...

Traci, is it possible that she misunderstood the date and or instructions. If I were her, I'd tell them this was totally unacceptable! I have NEVER. Heard of anyone having these two things done the day before....something doesn't add up, perhaps whom ever gave her the instructions, didn't know what they were talking about! And If I remember correctly, the radioactive dye has a short half life, meaning it leaves the body fairly quickly, leaving doubt it would still work as well, the following day...... like you said, the numbing agents wouldn't last that long....my wire was taped to my body....and at the surgery center the nurse helped me undress, so as not to disturb the wire...

My very first post on this board was " wire needle localization, whos's had this?". I was terrified....turned out not to be anything like I had imagined...I was thinking a coat hanger wire......lol.

    Let us know...

    Mine was done 30 minutes
Mine was done 30 minutes before surgery. In fact, it delayed my surgery because it took so long to get the wires in. Mine had to be put in from the underneath. What a sight to see: the tech hanging upside down and the doctor on her knees. I had a wire sticking out the top of my breast. Can't imagine going home with that!! It was very uncomfortable and weird feeling. I couldn't wait for the surgery to begin to get it out!! Please have your friend ask again if that's what is going to be done. If so, I would flatly state that there is NO way she is going to leave with it in there and go on with her day.Hopefully she is mistaken.

Thank you all for your input
As many of you've said, I do think my friend is mistaken about having the wire localization done the day before....but then, I also thought she was mistaken about having the lumpectomy and sentinel node biopsy done through one incision, and GrowingSoul responded to my post yesterday letting me know she had only one incision....so I'm clearly not ready to become an Expert Breast Surgery Consultant just yet! :-)

Chen, the wire localization is done by a radiologist before surgery, using mammography and/or ultrasound, to give the surgeon "runway lights" to find the tumor in surgery (at least that's how my radiologist explained it to me). If your tumor was right on the edge of your breast, or easily palpable, maybe your surgeon didn't need "bread crumbs" to find it?

I'll let you all know how this turns out,

    Sounds completely unfamiliar
Sounds completely unfamiliar to me Traci~ I had an ultrasound the very day I found my lump~ my GP sent me right over to the Breast Center. It was soooo obvious that I do remember them telling me this was not being done to rule out cancer, but to confirm it. My tumor was the size of a walnut and I could SEE it under my skin, not to mention feel it. So perhaps as you said, it wasn't a necessary procedure for me. Thanks for the info!

How well I remember the needles in my breast, bleeding as they took pictures. It was done in the am and my lumpectomy was done around 1 pm the same day.

    Day of for me
Interesting... I had wire localization done the day of the lumpectomy. I think I would have freaked if I had to go home with a wire sticking out of my breast. Also, the drugs would wear off and then the pain would start. Just curious, is your friend going to the same place you went for treatment?

Best of luck to your friend, Traci.

OMG! I can't even imagine having it done the day before!
I had this done the morning of my lumpectomy. It is surreal. At least is was to me. I was supposed to have been given Ativan pre, but I didn't get it. First they did the dye injection - bee sting! They inject quickly, then you rub so it doesn't just sit in one spot. I am a wimp and this hurt - not unbearable, but I am a wimp.

Then to the wire/torture room. Sitting in a chair that they wheeled up to a mammo machine where they put me in the vise, and then inserted the wire. Then off to await my surgery with this 4" wire sticking straight up out of my boob.

Really, I think I was in a daze - you know the one, like when you first get the dx - so all of this is clearly in my memory, but yet it doesn't seem real - or possible at all.

Advise your friend to insist that it be done the same day and that she is prescribed an antianxiety med.

And, like everything else, it's doable.

    I, too, remember it well...
Everything all of you describe is just how my Surgery Day went....Bella Luna, I had my surgery at a local hospital, West Hills Hospital, while my friend is having hers at USC. Of course, different hospitals have different procedures, but it just doesn't seem feasible to have the wire localization the day before.

I do believe my friend is correct that they're doing the sentinel node biopsy injection the day before, as I've heard several of you here talk about having that done. And they probably get all the paperwork, blood work, etc., out of the way, too.

For me personally, I'm glad my hospital did it all in one day -- it was the longest day of my life (14 hours at the hospital), but I think doing it over 2 days would be even more anxiety-provoking for me.

    Biopsy and lumpectomy
The biopsy was done and then 2 weeks later the lumpectomy/wire to mark location before surgery. Just as she did the ultrasound and put the wire in the breast she found another tumor. She wanted me off the ultrasound and then wanted to do a mammogram. She thought that would be better since she found the second tumor. She needed to mark that one too.I never looked at the wire TILL I was put on the mammogram then looked and saw 2 wires.I felt nothing either time.No pain whatsoever before I was taken to surgery.

The radiology doctor did my pre-op.

I think your friend heard wrong or misinterpeted things. I think that would be dangerous to go home with a wire in you.

wire inserted the same day as surgery
I had the same as everyone above, the wire was put in ultrasound guided, then I had to get a mammo to ensure it was in the correct place...yup that was fun...

Then I went for surgery later that same day...

I had my lymph node biopsy as a separate surgery since my results were not a confirmed cancer..just suspicios prior to the lumpectomy.

Anyway, I can't imagine going home with the wire in...even the marks they put on you for the nodes would wash off if you were to go home and come back the next day...I wonder if it's just the pre-op that they are doing...

    Best of luck...

    needle loc
    Traci,
It also depends on if the tumor is even visible in the Mammo and sometimes where it is located. Some needle locs are done in ultrasound also. I have never, in the 5 years I have worked as a mammo tech, heard of either hospital doing a needle loc the day before.

Mine was done the day before
I went to my surgeon's office, he used some Lidocaine and inserted 3 wires (very, very small in diameter) to localize the area using ultrasound. He also injected the dye. Did it hurt--like hell!!!!! I remember squeezing the nurse's hand so hard that she had my nail prints in her. Then they put tape over the wires and I went to the mammo room to do a mammo on that breast to make sure the wires were in the proper place.

I walked back into the waiting room where my husband was and said, "Well, that was gruelling!" I hope your friend has an easier experience.

Mine was done on the ground floor of the hospital in the nuclear medicine department at 10 AM, prior to my 1:30 PM surgery. I cannot imagine it being done the day before and having to go home that way, UGGGGGGGGGG!!!!!! Good luck to your friend, and prayers for her!
