Mixed results ending with more confusion after ct scan

Hello Everyone, Well Connie got the results of her ct scan with mixed results. Her radiation oncologists told her the ct scan was clear but to come back in 3 months for pet/ct scan. She seen her main doctor (surgeon) the next day and she said their was a small lymph node that concerned her a little. She mentioned doing a needle biopsy on the lymph node to check for scc. The last time Connie did a needle biopsy the lymph nodes (2) tripled in size in about 3 weeks. I mentioned this to Connie's doctor and she said she would order a pet scan for 6/21/11 which is 3 months after radiation ended 3/21/11. Now I'm thinking maybe Connie should have the needle biopsy and if positive for scc then it could be removed immediately with surgery. If we wait to have the pet scan and it shows up as a hot spot then a biopsy would be a necessity before surgery anyways.I was hoping we would be relieved after the ct scan but now it seems to have caused more worry. I would appreciate some help with this because I am not questioning Connie's doctors but I'm not sure this should wait 5 weeks and then end up getting a biopsy anyways. Best Regards, Homer & Connie

    Why do unnecessary surgery?
I had an MRI about two months after end of chemo and rads. My Stanford doc told me that I had one or two lymph nodes that were large/inflamed, but that was to be expected after what I'd gone through. Three months after end of chemo and rads, I got a PET/CT. It showed NO hotspots. They were just inflamed lymph nodes, and not cancerous.

The surgeon is probably not going to be the best source for recommendations on whether or not to cut out the node now. If the radiation oncologist says wait, I'd wait. And, needle biopsies are notoriously inconclusive.

Healing thoughts to you and Connie. Sounds like you are right on track.

    Hi Homer

You might want to get the CT report and disk and get another opinion. That is what I do and one reason why I am still here, the doctors don’t mind in fact some of them will encourage you to do so.

    Wishing you the best

    Difficult
Difficult times when your challenged with what to do next and with what is normally thought of with cancer is take care of it early as you can. So many times I have read many have waited and the next scan showed no change and lets keep an close eye on it. Our anxiety kicks in at these times and that makes us extremely impatient which is normal. Keep in mind that your radiation treatments continue for some time after they stop(approx. 6-9 weeks). For me my hot spots came up scar tissue.
Prayers going out for wisdom for both you and Connie. Maybe give it a week and talk to your two doctors again when you have had time to think about it and with it being older news you will be able to listen and ask more questions to help with your choices.

    2nd opinion
    Hi Homer,

That is great that the ct was clear!
I'm with Hondo on 2nd opinions. If there's ever a question about the next stage of treatment it is a good idea to get a 2nd or even 3rd opinion. It really helps with the decision making and prevents the "should haves" later. By the way; I had 3 needle biopsies that came back negative for cancer. It took a dissection to find it. Also had 5-6 ct scans that showed no tumors. It took an MRI to find those. So, I'm not real confident in needle biopsies or ct scans. Hang tough,

Deb, I am with you on this.

Deb, I am with you on this. I talked to Connie and she just doesn't want to do a needle biopsy right now. She has bad memories of the last one and she doesn't want to do anything to upset her lymph nodes, if that makes sense. They are all connected and she is afraid "all hell might break loose" if she messes with them. On Thursday we got great news "all clear" on Friday we're talking needle biopsy. Our celebration was short lived and we don't understand how 2 doctors from the same hospital and same team could give us such conflicting news. Either way, you responded within an hour of my posting this question. This site is "unbelievable". Thanks, Connie & Homer

    Hondo, We probably won't get
Hondo, We probably won't get a 2nd opinion because Connie's doctor has done so much for us their is not enough room on this page to list everything. I wouldn't want her to think we have lost confidence in her and I think she is just going the extra mile for Connie to make sure nothing gets overlooked. I never mentioned this before but Connie's medicaid was canceled last week and we were at MDA for an appointment and a social worker led us from the waiting room to a private room and told us we would not be seeing anymore doctors till we got the medicaid issue resolved. I reminded them that Connie applied and was excepted at MDA for 100% assistance and the only reason we applied for medicaid was because our advocate requested we do so. They told us that the day Connie received medicaid our assistance was canceled and that we would have to re-apply if we wanted assistance. It took over a month to be accepted for assistance the 1st time so we headed for the social security office to see why her medicaid was canceled. The social security office said Connie was placed on social security disability and would now start receiving a check for $350 a month and this added to my income made her ineligible for medicaid. This would place her on medicare and she would have to wait 24 months before she could actually get medicare. That night I wrote her doctor and explained our situation and she wrote the head of the social worker department and told them that we would not be re-applying for assistance and that they needed to get the situation resolved pronto. So the next morning we got a phone call at 8am asking us to come back to the hospital and to make a long story short we got assistance so when doctors help in that way we could never 2nd guess her. Believe it or not though Connie's medicaid was restored and we ended up with medicaid coverage again. Crazy huh! Your friends, Homer & Connie

John, We have decided to wait and do the pet scan because Connie doesn't want to do the needle biopsy. So hopefully we are making the right decision. Connie's doctor is just trying to be on the safe side on this. We appreciate every bit of advise we get here and will continue posting our questions (problems). Thank you, Homer & Connie

It never hurts to get a
It never hurts to get a 2nd/3rd opinion. My ENT actually suggested I get another opinion after a recent PET/CT scan showed a hot spot.

I have had a hot spot show up on a 6 month scan and also a 9 month scan. Since it has gotten smaller on the most recent scan, they believe it is inflammtion. The 2nd opinions I sought out suggested an MRI, which was clear.

As Deb and Bob mentioned, a needle biopsy may not be conclusive. As it was explained to me: Particularly after rads/chemo, the cancer may no longer be a solid mass, but something more like swiss cheese. The needle biopsy may miss the cancer because of this. Therefore a negative result from a needle biopsy will not be 100% conclusive there is no cancer present. Cheers
